Which forms do I need to download to play at Action Paintball?

Players in NSW aged between 16 & <18 MUST have a signed 16+ Parental Consent Form plus a signed 16+ Indemnity Form.

Players in NSW aged 18+ MUST have a signed 18+ Indemnity Form.

Players in WA aged between 12 & <18 MUST have a signed 12+ Parental Consent Form plus a signed 12+ Indemnity Form.

Players in WA aged 18+ must have a signed 18+ Indemnity Form.

These are the only required forms to play at Action Paintball. They will also be available at the field on the day, however you can speed up the processing of your group by downloading and filling them out before you play. All other forms can be downloaded as desired.
