Action Paintball Games Consent & Info Forms: |

The parental consent form must be filled out and signed by a parent/guardian and
brought on the day when playing at Action Paintball Games Sydney or Perth.
If ALL players are under 18, a parent/guardian must remain on site whilst participants
are playing.
Players 16 years and over, but under 18 years in Sydney must provide a signed,
16+ Indemnity Form and a signed
16+ Parental Consent Form (available above).
Players 13 years and over, but under 18 years in Perth must provide a signed,
13+ Indemnity Form and a signed
13+ Parental Consent Form (available above).
18+ players must also bring photo ID in addition to filling out an
18+ Indemnity
form.
